Background

The Government Performance and Results Act of 1993 (Public Law 103-62) required federal agencies to set goals, measure results, and report progress to Congress. The Maternal and Child Health Bureau (MCHB), with the assistance of an external committee, developed measures according to the four levels of the maternal and child health service pyramid (i.e., basic, enabling, population-based, capacity/infrastructure), sharing accountability for planning and evaluation with states through the annual state application for Title V funding.

Impact

The law strengthened the health-improvement goals of the Omnibus Budget Reconciliation Act of 1989 in that states were required to respond to federal guidance to establish strategic plans and report on MCHB-specified performance measures. The Title V Information System (TVIS) database was developed to capture financial and program data for state and national use. TVIS standardized data collection, eased states’ burden for reporting, and permitted broad analysis of data.

From 1994 to 2003, national performance measures, health-systems-capacity indicators, national outcome measures, and health-status indicators were developed, including two specific to oral health:

  • National performance measure: Percentage of third grade children who have received protective sealants on at least one permanent molar tooth
  • Health-systems-capacity indicator: Percentage of children ages 6–9 who are eligible for Early and Periodic Screening, Diagnosis and Treatment Program service and who have received any Medicaid dental services during the year
