State Dental Summits:
An Evaluation - Executive Summary
This
executive summary describes the findings and recommendations
of an evaluation of 17 state dental (oral health)
summits held between April 2001 and December 2002
and funded through cooperative agreements with
the Association of State and Territorial Dental
Directors. Evaluation topics include (1) whether
these oral health summits were successful in moving
forward oral health agendas, (2) how the federal
agencies supporting these summits can better understand
how funds were used and how the funding eventually
affected the oral health activity in the states,
and (3) how assistance to oral health stakeholders
in the states can be provided as they plan future
oral health summits. Summary sections include discussions
of evaluation design and methods, survey results,
and recommendations for future oral health summit
planners and the funding agencies.

These

This preliminary report describes the oral health summit
held on April 7, 2003, in Augusta, Maine. The purpose of the

of and access to oral health services and improve oral health
in Maine. Contents include background information on the project
and stakeholders, presentation summaries, workgroup summaries,

This report describes the July 2002 meeting of the Coalition for New Hampshire

the Oral Health for All New Jerseyans
This document
contains the agenda and selected papers presented at the first
New Jersey Oral Health Summit held on September 12, 2001, in Princeton,
New Jersey. Participants discussed topics such as access to oral
health in New Jersey, the definition of oral health and oral disease,
and the impact of dental insurance. Other topics discussed included
increasing Medicaid dental reimbursements to providers, water fluoridation,

Oral Health in New York State
This
report of the 2001 New York State Children's Dental
Summit presents facts and figures on children's oral
health in New York and strategies that state government
officials, health professionals, and children's advocates
could consider as part of an overall plan to address
the disparities that exist in oral health. Strategies
are presented in four categories and include the
following initiatives: school-based oral health,
dental workforce, access to dental care, and professional
and public education. An executive summary, conclusion

This
participant folder contains conference materials including the agenda,
seven speaker biographical sketches with brief descriptions of their

and Child Oral Health Resource Center. Conference topics include
an overview of oral health in America and the District of Columbia,
financing and workforce considerations, discussions on barriers to
care and developing strategies for an oral health plan, and the Pipeline

Oral Health Leadership Summit: Rebuilding on a Framework for
Improving Oral Health in the District of Columbia
This report describes the summit held July 18, 2003 in Washington, DC, as part
of a strategic response to nationwide calls for oral health planning and improvement.
Topics include oral health policy development, oral health quality assurance,
and initiating linkages with dental and medical providers as well as the dental
community. Also discussed are issues surrounding low Medicaid reimbursement
rates, bureaucratic administrative requirements, lack of uniformity and methods
to rebuild the dental infrastructure in the District of Columbia in order to
facilitate greater access to dental services, particularly for children with
unmet needs. Report contents include an overview of oral health in the District
of Columbia, summit recommendations, a list of speakers, and the summit agenda.
Statistical data is provided throughout the report.
Shaping
the Future of Oral Health: From Cavities to Oral Cancer
This folder contains materials for participants at the Second Oral Health Summit
held on November 30, 2005, in Washington, DC. Agenda topics include the Colgate
Bright Smiles Bright Futures Program, school-based oral health programs, oral
health coverage and access to care, oral health programs and the community,
oral health services for specialized populations, and local initiatives to
address oral health disparities. Contents include the meeting agenda, speaker
biographies, two articles, a report entitled "Improving the Oral Health
of African American Males", an information sheet on coalitions for oral

Report
This report summarizes the Second Oral Health Summit held on November
30, 2005, in Washington, DC to address the need for an integrated
oral health delivery system and health plan for District of Columbia
(DC) residents with unmet needs and to address disparities in oral
health as it affects the dental access of children. Report contents
include an introduction to the needs and oral health status of DC,
an outline of summit sessions and the luncheon presentation, recommendations,
and a review of activities the DC Oral Health Division is currently
doing and future plans. A copy of the summit agenda is included.

This
document contains the proceedings from the oral health summit held
on September 22, 2000, in Baltimore, Maryland. The purpose of the
summit was to enhance awareness of the oral health of children in
Maryland and related access issues. Topics discussed included financing,
partnerships, and the replication of best practices already in place
in the state. Appendices include the summit agenda, presentation
slides, and the text of the keynote speech.
Pennsylvania
Dental Summit
This
notebook was developed as background material for the Pennsylvania
dental summit held on May 10-11, 1999, in Grantville, Pennsylvania.
Five workgroups organized around children, special needs populations,
managed care, and provider inducements and quality met to discuss
accessibility issues and to make recommendations. Contents of the
notebook include the following: (1) a statement of the conference's
vision, goals, and objectives; (2) Pennsylvania regulations pertaining
to the Medical Assistance Program and dental services; (3) a Medical
Assistance fee schedule; (4) copies of invoice forms; and (5) statistical
charts. Several related articles are also included in the notebook.
Beyond the
Barriers: Building Innovative Networks of Care -- Oral Health
Summit
This notebook was prepared for the oral health summit held on
November 17, 2000, in Lancaster, Pennsylvania. The summit was convened
to address barriers to oral health care in the county. Conference
materials consist of objectives, an agenda, and fact sheets that
discuss oral health in general and the specific situation in Lancaster
County. Follow-up materials, including two issues of a newsletter
and proceedings from the conference, are included in the notebook.
2001 Dental
Summit: Meeting the Challenge at All Levels
This notebook was developed as background material for the dental
summit held on November 1-2, 2001, in Carlisle, Pennsylvania. The
purpose of this summit was to provide a forum for key stakeholder
groups to collaboratively develop recommendations to improve access
to and utilization of dental services for all recipients in both
the medical assistance fee-for-service delivery system and the medical
assistance managed care delivery system. The notebook provides information
on dental summit updates, demographic statistics, enrollment figures,
dentist/population ratios, and medical assistance bulletins. It also
includes a list of state dental contacts.
Oral Health Strategic Plan for Pennsylvania
This report describes Pennsylvania’s strategic plan to address
the importance of oral health and the problems that exist in the
state. The report is intended to serve as a catalyst for further
development and implementation of action steps to improve oral health
in general, with special attention to areas in which oral health
disparities are most evident. Report sections include (1) assessment
of the nature and magnitude of oral health problems in the state,
(2) reducing financial barriers to oral health access and treatment,
(3) prevention and treatment of oral diseases and conditions, (4)
workforce issues in the state, (5) special populations at greatest
risk for poor oral health, and (6) fluoride and water fluoridation.
Planning tables in each section offer short- and long-term recommendations.

Virginia Statewide Dental Summit Proceedings: September 29-30, 2003
This report summarizes the proceedings of the Virginia Statewide
Dental Summit held on September 29-30, 2003, in Richmond, Virginia.
The report includes final presentations and recommendations from
the breakout sessions as well as the 2004-2010 Oral Health Plan for
Virginia. The report is intended to complement the materials included
in the participant folder. Report content includes a commitment form
for participation in recommendations set out in the state oral health
plan, an order form for proceedings on DVD, notes from the discussion
held by the Prevention and Education Group, and a packet of slide
printouts on oral health in Virginia. Also included is the oral health
plan for Virginia that provides recommendations for statewide education,
legislation, administration, prevention, and data collection and
surveillance measures.

This participant's notebook includes materials for the Virginia Statewide Dental
Summit held on September 29-30, 2003, in Richmond, Virginia. The purpose of
the summit was to develop a plan to address the oral health needs of Virginians
in the areas of prevention, education, access, and infrastructure. The notebook
includes the meeting agenda, biographical sketches of presenters, a participant
list, a compendium from the American Dental Association on state innovations
to improve oral health care access for children from families with low incomes,

Improve Children's Access to Medicaid/CHIP Oral Health Services
This notebook contains information from the Region 4 conference
held on March 3-4, 2000, in Cary, North Carolina. Topics discussed
included (1) access to dental care for children; (2) state programs
to improve access to care in Indiana, Georgia, and North Carolina;
(3) an overview of the State Children's Health Insurance Program,
with an emphasis on North Carolina; (4) the roles of various dental
professionals in improving access to care; and (5) the role of public-private
partnerships and litigation in improving access to care. Presentations
on specific states provided statistical information about poverty,
racial factors, and the number of dental professionals providing
care to Medicaid-enrolled children.

the Solution to the Problem: Dental Access for Alabama's Children
-- Alabama Dental Summit 2001 Conference Proceedings
These
proceedings report on the dental summit held on December 6-7, 2001,
in Montgomery, Alabama. The purpose of the summit was to discuss
the gaps in the dental care infrastructure, barriers to good oral
health, and the development of specific recommendations to incorporate
into the state's Strategic Oral Health Plan. The proceedings include
summaries of seven presentations, information on the workgroup facilitators,
and workgroup summaries and recommendations in the areas of legislative
and regulatory change, identification of funding resources, building
public awareness, and surveillance and monitoring system development.

Georgia Oral Health Summit
This notebook contains materials provided to participants in the oral health
summit held on September 20, 2002, in Macon, Georgia. Contents include a list
of sponsors and planners for the summit; information on program presentations,
issues, and solutions; maps; an evaluation of the Georgia Oral Health Prevention
Program and the Georgia Access to Dental Services Grant; state oral health
information; and national oral health resources. Topics include an overview
of the importance of good oral health; a review of oral health in Georgia,
including state initiatives in planning, needs assessment, prevention, and
treatment for all Georgia citizens; state program evaluations; and oral health
professionals' participation in state programs. The resources section includes
information from Healthy People 2010, the Surgeon General, and the Centers

This report summarizes the proceedings of the Kentucky Dental
Access Summit held on May 24-25, 2001, in Lexington, Kentucky. The

appendices. The appendices include an agenda, presenter biographies,
a pre-summit questionnaire and responses, and lists of the participants
and planning committee members.

Dental Access Summit: Notebook
This participant notebook for the Kentucky Dental Access Summit held on May
24, 2001, in Lexington, Kentucky consists of eight sections that include background
facts, articles, papers, and other print items. Section 1 contains federal
information on trends, data, and background papers. Section 2 includes Kentucky
statewide statistics, surveys, and goals. Section 3 provides information on
the presenters, including background papers or summaries from keynote speakers.
Section 4 contains items on legislative issues. Section 5 is devoted to public
awareness and education and to initiatives to raise oral health education and
awareness. Section 6 discusses dental service delivery models. Section 7 contains
initiatives from dental educators/dental schools on dental education. Section
8 provides Web sites, addresses, and telephone numbers for more information.
The notebook also contains separate copies of the Oral Health America brochure,
the executive summary of Oral Health in America: A Report of the Surgeon General,

Kentucky Smiles: A Lifetime of Oral Health—Statewide
Oral Health Strategic Plan
This report outlines a strategic plan for improving and enhancing
the oral health and general health status of Kentucky citizens. Topics
include oral health surveys for children and adults; strategic plan
mission, vision and values statements; a situation assessment of
strengths, changes, opportunities, weaknesses, and threats; advocacy,
economic development, funding, partnerships and collaboration, prevention

North Carolina Oral Health Summit Access to Dental
Care: Summit Proceedings and Action Plan
This report summarizes an April 8, 2005 meeting to identify
potential strategies to improve dental care access to North Carolina's
Medicaid recipients. Contents of the report include a listing of
summit participants; a brief history of previous meetings and efforts,
and an overview of the problems in 2005. Topics include (1) increasing
dentist participation in the Medicaid program; (2) increasing the
overall supply of dentists and dental hygienists in North Carolina,
(3) focusing on underserved areas and populations; (4) increasing
the number of pediatric dentists and expanding the provision of preventive
dental services to young children; (5) training dental professionals
to treat patients with special needs and designing programs to expand
access to dental services; and (6) educating Medicaid recipients
about the importance of ongoing dental care, and developing programs
to remove non-financial barriers to the use of dental services. Each
topic is summarized, with previous recommendations reviewed, and
proposed recommendations provided.

This 3-ring binder contains materials for participants in the oral health summit
held on April 8, 2005, in Chapel Hill, North Carolina. Contents include the

Illinois' response to the Surgeon General's report on Oral Health in America

This manual highlights topics discussed at the conference, A Smile for
All Ages -- Action for Oral Health, held on June 22-23, 2000, in Lansing, Michigan.
The manual is divided into sections corresponding to the conference sessions
and covers the following topics: (1) a Michigan oral health program directory
listing detailed information about local agency oral health programs, (2) summary
findings from the U.S. Surgeon General's workshop on children and oral health,
(3) local health departments' perspectives, (4) oral health promotion goals

Ohio Access to Dental Care Summit, November 14-15, 2001: Final
Report
This final report describes the summit held on November 14-15,
2001, in Columbus, Ohio, to discuss the recommendations of the
Ohio Director of Health's Task Force on Access to Dental Care.
Topics discussed included increasing awareness of the problem
of access to dental care and encouraging local action and advocacy,
providing communities with the opportunity to gain the skills
needed to develop dental care access initiatives, creating a
forum for networking and sharing experiences, and catalyzing
new partnerships to address the access to dental care problem.
The report is divided into three sections: program overview,
program accomplishments, and evaluation.

Oral Health Summit 2002
This report summarizes the first statewide oral health
summit, held on December 6, 2002, in New Orleans, Louisiana.
The summit was designed to assemble oral health leaders and
policymakers to address access to oral health care, infrastructure,
education, and financing of oral health care. Discussion focused
on the oral health needs of children in general, then specifically
on the needs of children enrolled in Head Start. The report
provides information on the oral health needs of children including
children enrolled in Head Start, pregnant women, adults, the
elderly, and individuals with special health care needs. The
report concludes with a discussion of oral health provider
participation in Medicaid.
Louisiana's
Oral Health Summit: Summary of the Proceedings: Another Bite
of the Apple
This brief report summarizes the second oral health summit
held on December 3, 2004, in Baton Rouge, Louisiana. Brief overviews
of the sessions are included, along with a summary of responses to
the summit evaluation. Results of facilitated group discussions on
the following topics are provided: (1) ways to increase the number
of dentists currently providing services to Medicaid patients, (2)
ways to increase access to services for special needs populations,
and (3) challenges in providing oral health care in rural areas.
The summary concludes with a “wish list” of other ideas
presented during group discussions.

Mexico Dental Health Summit 2000 (executive summary)
This
report summarizes the discussions at New Mexico's Dental
Health Summit 2000 held in January 2000 in Albuquerque,
New Mexico. Five major topics were reviewed: (1) health
promotion and disease prevention, (2) supply of and demand
for dental professionals, (3) access to care and infrastructure,
(4) dental education and training, and (5) finance, Medicaid,
and insurance issues. A two-page executive summary is
also available.

This report describes the roundtable and breakout sessions for the Second Oral
Health Access Summit held on April 25-26, 2002, in Las Cruces, New Mexico.
Session topics included finance and legislation, border health, child oral
health and prevention, work force and system capacity, and New Mexico's oral
health plan.

Children's Oral Health Summit: A Challenge for Change --
Final Report
This
report summarizes the events that occurred at the Children's
Oral Health Summit held on January 25, 2002, in Oklahoma City,
Oklahoma. This summit initiated one of the first steps toward
communicating and planning Oklahoma's approach to improving oral
health services for children with the greatest need. This document
includes an agenda, a narrative report, and rosters for the planning
committee, speakers, and attendees.

Health Initiative Summits 1999-2000
This
document presents reports from summits held during
2000 in Iowa, Kansas, and Missouri. These summits
addressed the challenges associated with access to
oral health in the respective states. In Iowa, the
problem that surfaced was dentists' low rate of participation
serving Medicaid recipients and other underserved
groups. An oral health coalition and several committees
were formed to address the issues raised, and a resolution
was sent to the legislature. In Kansas, several initiatives,
such as an initiative to institute community water
fluoridation, were launched and other efforts were
proposed. In Missouri, the Coalition for Oral Health
Access and committees were formed and charged with
identifying specific strategies to address barriers
to oral health access.

the underserved. The report includes information on federal,
state, and local assistance programs. The report also includes

State Oral Health Forum
These
participant materials include a notebook binder and
two videotapes from the Nebraska State Oral Health
Forum, held on April 24-25, 2004, in Lincoln, Nebraska.
The binder, titled Oral Health Tool Kit, is divided
into five main sections. Section 1 contains report
and findings; section 2 contains data, maps, and contacts;
section 3 contains policies and guidelines; section
4 contains education, prevention, and access materials;
and section 5 contains resources and references. Also
included are a brochure about the importance of visiting
the dentist during pregnancy, a booklet that lists
free dental education materials, and a list of forum
participants. The first videotape describes several
programs and protocols that aim to reduce tooth decay
in children. Contact information to learn more about
the programs and protocols presented in the videotape
is provided. The second videotape contains presentations
from the conference.

This participant kit is from a summit held on November

Report
This report describes a summit held on November 19, 2004, in Denver,
Colorado, to highlight oral health in the state, to create and implement
a plan to improve oral health, and to involve more individuals and
organizations. Topics include advocacy participation, developing
an oral health plan, developing and participating in local coalitions,
promising practices, promoting health policy, financing, workforce
issues, and systems of care. Appendices include samples of summit
handouts, letters of invitation, and additional background information
on oral health surveillance, goals, and needs in Colorado.

This report describes an action plan to improve the oral health and
general health of all Coloradans. It focuses on six major topics
areas: (1) financing, (2) health promotion, (3) policy and advocacy,
(4) promising practices, (5) systems of care, and (6) workforce.
Related outcomes and strategies are provided for each topic area.

Montana Dental Summit: Access for the Underserved -- Meeting Proceedings
The
first Montana Dental Summit was held on November 18, 1999, in Helena,
Montana.This
report summarizes the discussions, presentations, and comments of
the summit participants, including a combination of factual presentations,
current perceptions about the status of the oral health problems
in Montana, and the comments shared that led to strategies and next
steps. Also included is an outline of the summit purpose and preparation,
along with small group reports on strategies to address key issue
areas.

This
participant folder provides summit participants with
background information on the importance of oral
health and the need for a state-wide strategy to
ensure access for the underserved population of Montana.
Contents include two periodical issues, seven fact
sheets, a brochure, and one article on issues of
homelessness, child oral health, and lack of access
to oral health services. Also included are introductory
materials, the meeting agenda, a series of brief
handouts, information on Montana children's health
insurance plan dental benefit, the Health Resources
and Services Administration handout on Addressing
Unmet Oral Health Needs and Disparities for the Underserved,
and a progress review of the Healthy People 2000
on oral health.

Dental Access Strategies: Making it Happen
This document provides an agenda and related materials for an interim Montana
Dental Summit held on July 28, 2000, in Helena, Montana. It lists the workgroup
members for data collection, finance and policy development, public education,
and work force analysis. It includes the purpose and short- and long-term strategies
of each workgroup, as well as instructions and criteria for completing the
work, and a description of the Montana Dental Action Plan process. Also included
are results of a community health center dental clinic survey, an article from
Rural Health News (vol. 6, no. 3), "States Respond to Rural Dental Crisis," and
the executive summary of the 2000 report of the Surgeon General about oral
health in America.

This action plan is the final copy of the Montana Dental Access Coalition proposals
that were reviewed by a Children, Families, Health, and Human Services Legislative
Interim Committee. The action plan and proposals are part of an effort to improve
oral health and access to dental care in Montana, particularly for the underserved.
The committee's recommendations following the review are included.

This participant folder is from the second Montana Dental Health
Summit held on November 2, 2001, in Helena, Montana. Materials include
an agenda, a list of remaining strategies from Dental Summit I, a
list of priority issues for the summit compiled by the Montana Dental
Association, information about oral health issues in various parts
of Montana, a sheet of Montana oral health referral resources, a
form for requesting to be added to the mailing list, and a copy of
Montana's Healthy People 2010 state plan. The following U.S. Department

Dental Summit II: Building Community Partners
This final report describes the second Montana Dental Health
Summit held on November 2, 2001, in Helena, Montana. The focus of
the summit was to engage communities in improving oral health services
in the state. Reports of efforts to improve access from the federal,
congressional, regional, state, and local levels were shared, along
with current work force data from the Montana Dentist Survey, 2000.

This
information package was developed as background material for the
dental summit held on April 14, 2000, in Bismarck, North Dakota.
It includes copies of slides from presentations about the federal
Health Resources and Services Administration/Health Care Financing
Administration Oral Health Initiative, oral health in North Dakota,
and an overview of (1) the Indian Health Service Program, including
statistics about dental caries in Native American children; (2) Medicaid
in North Dakota, with statistics about dental fees and enrolled providers,
and (3) the North Dakota Health Tracks program. Also included are
comments on access and medical assistance, a paper about the dental

This document contains the proceedings from the dental summit
held on April 14, 2000, in Bismarck, North Dakota. The purpose of
the summit was to engage North Dakota in the federal Health Resources
and Services Administration/Health Care Financing Administration
Oral Health Initiative; to develop and consider policy changes as
well as other strategies to increase access to dental services, particularly
for the underserved; and to follow up on dental issues generated
at the Children's Health Care Needs Workshop held in August 1999
in North Dakota. Priority issues included lack of dental manpower
for the state; financing and resources required to support manpower
improvement and dental services in the state; and challenges in providing
services to special populations.

Utah. The goal of the summit was to develop and reach a consensus on short-term
strategies and a long-term work plan to improve the oral health status of residents
in Utah. The report details participants' efforts in three work groups: policy
and funding, prevention and education, and access. The appendices present the
views of the three keynote presenters on the Health Care Financing Administration/Health
Resources and Services Administration Oral Health Initiative; national, state,
and local responses to oral health; and current efforts to improve oral health
in Utah.

This
document reports on the Nevada State Health Division Strategic
Meeting of Oral Health Stakeholders held on January, 24, 2002,
to evaluate the state oral health program. Also discussed were
issues pertaining to establishing, strengthening, and expanding
the capacity of Nevada to plan, evaluate, and implement oral
disease prevention and health promotion programs. Section contents
include past oral health assessments, identified gaps in oral
health in Nevada, the vision statement, plan development, remaining
issues, and evaluation of goals and objectives. The report
