This toolkit provides background for policymakers on the importance of developing community-based oral health systems focused on promotion, prevention, and treatment services. It also provides recommendations for legal, regulatory, and reimbursement reform to improve oral health in underserved populations. Contents include a series of eight policy briefs on topics such as child and perinatal oral health, coverage, community water fluoridation, the safety net, and state infrastructure.

This report describes the status of children's oral health in California, based on the results of a needs assessment conducted in 1993–94. The report presents the rationale for the assessment, describes its design, and presents and discusses the findings concerning infants, school-age children, and adolescents. The document also reviews existing state programs, specifically considers the need for fluoridation programs, and concludes with recommendations for improving the oral health of children in the state.

This report presents the results of a survey of the 50 states and Puerto Rico concerning the status of each state's Medicaid dental managed care program. Of the 31 respondents, 14 states indicated having some or all individuals eligible for Medicaid covered under a dental managed care program; 9 other states are studying the feasibility of such a program. The survey addressed eligibility requirements, utilization, covered services for children and for adults, types of providers, reimbursement methods, and capitation fees of Medicaid dental managed care programs.

This is the California Geographic Managed Care Dental Model by which contractors are licensed with the State Department of Corporations. It covers definitions, general provisions of the contract, contract term and termination, duties of the contractor, duties of the state, eligibility and enrollment, payment provisions, standards, financial reporting policies and procedures, and confidentiality requirements.

This report provides an overview of the Dental Sealant and Education Initiative proposed by the Children's Dental Center. The proposed initiative involves a highly collaborative and multifaceted effort to reduce the incidence of preventable dental disease in low-income school-age children in Southern California. The initiative would use proven technology to reduce tooth decay and would model cost-effectiveness by using a community-based mechanism for delivering the technology. The report addresses the initiative's goals, principles, and factors that would contribute to its success. An implementation schedule is included.

This report summarizes the findings and recommendations from an assessment of oral health needs among California's children in 1993–94. The assessment revealed a virtual epidemic of untreated oral diseases in the sample of more than 6, 600 California children ranging in age from preschool through grade 12. The report presents a strategic plan to achieve oral health and includes an appendix of project support personnel and schools participating in the assessment.

The report first defines and describes this minority group and then identifies and analyses major features of health status. Other sections of the report examine barriers to accessing quality care including financial, language, cultural, structural, and systemic, and also enabling dynamics, such as geographic clustering, ethnic specific networks, communities strongly influenced by local and national leaders, emphasis on extended family, and international relations. Future projections for this minority population and recommendations for policy, service, research, education and training, and leadership conclude the report.

This summary of 15 state reports discusses (1) oral health care issues for Medicaid-enrolled children, (2) the children at highest risk for dental caries, (3) barriers to provider participation, (4) Medicaid payments for oral health care in relation to state Medicaid expenditures, (5) states' response to children's oral health needs, (6) lack of access to oral health services for Medicaid recipients, and (7) the result of untreated oral health problems. Potential strategies for an integrated solution are discussed. Specific reports are provided for Arizona, California, Colorado, Connecticut, Hawaii, Iowa, Maryland, Montana, New Hampshire, North Dakota, Ohio, Oregon, Rhode Island, Virginia, and Washington.
