  • Fluoride varnish manual provides information for health professionals on fluoride varnish application, as well as parent information and consent forms (in English and Spanish).
  • The Head Start oral health survey was conducted to amass oral health data on children ages 3 through 5 enrolled in Head Start. The survey collected prevalence data on early childhood caries, caries experience, untreated tooth decay, and the need for urgent oral health treatment. The 2004 summary includes the following main sections: (1) methods, (2) procedures, (3) supply list, and (4) recommendations. Four appendices include the initial letter to parents, a parental consent form, a treatment urgency form, and a letter to parents explaining the screening. All letters and forms are in English and Spanish. The 2004 report includes the following main sections: (1) sampling and methods, (2) key findings, (3) statewide results, (4) results by region, (5) results by race/ethnicity, (6) results by age, and (7) access to care and Healthy People 2010. Statistical information is presented in tables and figures throughout the report. Additional tables and figures appear in an appendix.
