4.4 How Can Professionals Record Oral Health Information?

Like all health information, oral health risk assessment results, clinical findings from the screening, and recommended follow-up should be recorded in an infant’s or child’s health record.

If there is more than one professional conducting oral health risk assessments and screenings, it is important that all professionals share information and that they collect and record information using a consistent format.

A copy of the Oral Health Risk Assessment, Screening, and Follow-up form should be filled out each time an infant or child receives an oral health risk assessment or screening, and the completed forms should be kept in the infant’s or child’s health record.
